Or email me at with any questions. /n /n As an Elara Caring Clinical Team Manager, you’ll contribute to our success in the following ways: /n /n - Provides oversight of all patient care services and personnel including: making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. /n - Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities. /n - Ensures the timely start of patient care within recommended regulatory guidelines. /n - Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, and the physician and primary care physician (as appropriate). /n - Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness. /n - Integrates orders from all relevant physicians involved in the plan of care and ensures the orders are approved by the responsible physician. /n - Conducts regular OASIS reviews for all patients. /n - Meets with all direct care team members on a regular basis to provide guidance, coaching and communicate information related to specific issues or organizational changes Initiates hiring requests, interviews candidates and recommends hiring of direct care staff. /n - Communicates frequently with the Branch Director or Administrator on key performance metrics. /n - Remains available for field work, as needed. /n - Physically demanding, high stress environment /n - Performs other duties/projects as assigned. /n /n /n Why Join the Elara Caring mission? /n /n - Work autonomy and flexible schedules /n - 1:1 patient care /n - Supportive and collaborative environment /n - Competitive compensation package /n - Tuition reimbursement for full-time staff and continuing education opportunities for all employees /n - Comprehensive insurance plans for medical, dental, and vision benefits /n - 401(K) with employer match /n - Paid time off, paid holidays, family and pet bereavement /n - Pet insurance /n /n What is Required? /n /n - Associate degree in a Nursing related field required /n - 2 years home care experience as a registered nurse in a Home Health or Hospice environment /n - 1 year of supervisory experience preferred /n - Current, unrestricted RN license valid for the state of work /n - Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office /n - Able and willing to travel within branch/office coverage area. /n - Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. /n - Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment /n /n /n The base salary for this position is $90,000 to $110,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, internal equity and production points. /n This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings.
