Position Summary

The Pay-Per-Visit Registered Nurse (PPV RN) conducts client assessments, develops and updates plans of care, completes supervisory visits, and performs discharge evaluations for clients receiving non-medical home-care services. The RN helps ensure services remain appropriate, safe, client-centered, and consistent with company policies and applicable Georgia requirements.

This is an independent contractor position. Visits are offered based on client needs, location, and availability; no minimum number of visits or hours is guaranteed.

Primary Responsibilities

• Conduct initial client assessments and start-of-care visits.
• Evaluate clients’ functional abilities, personal-care needs, home environment, and safety risks.
• Develop and update individualized care plans and task instructions.
• Conduct required reassessments and supervisory visits.
• Evaluate whether caregivers are following the established care plan.
• Complete caregiver competency or skills assessments when assigned.
• Identify and report changes in client condition, safety concerns, suspected abuse, neglect, exploitation, or other reportable incidents.
• Communicate relevant findings and recommendations to agency management.
• Provide client and caregiver education within the RN’s professional scope.
• Complete discharge assessments and documentation when services end.
• Maintain client confidentiality and comply with HIPAA requirements.
• Follow infection-control, incident-reporting, emergency, and documentation procedures.
• Practice within the scope of the agency’s non-medical home-care services.

Documentation Requirements

• Complete and submit all required documentation in AveeCare before leaving the client’s home whenever reasonably possible.
• Ensure documentation is accurate, objective, complete, and submitted within the required timeframe.
• Notify agency management immediately if an emergency or technical issue prevents timely submission.
• Correct incomplete or rejected documentation promptly.
• Never falsify, copy, pre-chart, backdate, or alter client records improperly.
• A visit may not be approved for contractor payment until the required documentation has been completed and accepted.

Pay Structure

Initial Assessment/Start of Care

$160 per completed visit

Reassessment

$110 per completed visit

Supervisory Visit

$65 per completed visit

Discharge Assessment

$90 per completed visit

Minimum Qualifications

• Active, unrestricted Georgia Registered Nurse license.
• Current CPR/BLS certification.
• Current TB screening and required health documentation.
• Minimum of one year of nursing experience preferred.
• Home care, home health, geriatric, disability-care, or community nursing experience preferred.
• Current professional liability insurance.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Ability to travel throughout assigned service areas.
• Ability to complete accurate electronic documentation independently.
• Successful completion of all required credential verification, screenings, and company onboarding requirements.

Professional Expectations

The RN contractor must demonstrate professionalism, reliability, sound clinical judgment, confidentiality, respectful communication, and timely completion of accepted assignments. The contractor may accept or decline offered visits but is expected to complete any accepted assignment or provide timely notice when unable to do so.
