Nurse Practitioner/Nurse Care Manager

Role Overview This position is a hybrid opportunity in Riverside, requiring 60% on‑site patient care and 40% case management. Responsibilities Perform comprehensive health assessments, including history taking and physical examination. Complete review of medicines and related instructions. Review home safety and psychosocial needs. Fulfill documentation needs for STAR and HCC measures. Obtain and document all pertinent information in patient charts and case management notes on the same day as the visit. Include medications and end‑of‑life preferences and drivers of preventable hospital admissions. Instruct patients, caregivers, and families to call you prior to accessing medical care, subject to life‑threatening emergencies. Maintain patient safety and confidentiality using universal precautions. Prescribe or order procedures, treatments, and medications within scope of practice. Develop a high level of trust and intimacy with patients and families. Skills, Experience & Education CA Licensed Nurse Practitioner. 1+ years of experience in primary care. Compensation & Benefits Base Salary: $120,000 - $150,000 per year. Additional compensation may be awarded based on education, experience, and qualifications.
