Intensive Case Manager- Day Shift

Job Overview New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, a 24‑hour help line, a mobile response team, and outpatient programs across Indian River, Martin, Okeechobee, and St. Lucie counties. Position Summary New Horizons is a nonprofit agency seeking a caring and compassionate Intensive Case Manager to provide medically‑necessary quality intensive case management services to individuals with severe and persistent mental illness. Responsibilities Engage in staff hours of service for assigned individuals at a rate of 115 hours per month. Provide direct and indirect services to maximize revenue generated by NHTC within ethical principles and good business practices. Complete TCM assessments within the first thirty days of new admission. Monitor client progress toward treatment goals across all service modalities. Coordinate individuals' care both within and outside NHTC. Prepare written documentation for each service conducted. Develop written service plans in collaboration with the client, family, and other professionals. Coordinate referrals, admissions, discharges, and transfers between service modalities. Assist individuals with obtaining benefits (e.g., social security, medical insurance, food stamps, housing subsidies) and monitor their financial status. Provide timely and effective crisis intervention per the program's standards and protocol. Provide on‑call emergency service on a schedule for after business hours, seven days a week. Provide transportation as needed while adhering to agency transportation policy and procedures. Perform miscellaneous duties as instructed. Requirements Bachelor's degree from an accredited university or college with a major in one of: counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human service field focused on the study of human behavior and development. Minimum of one year of experience working with adults experiencing serious mental illness. Three years of experience if the degree is not in a human services field. Case managers must obtain CBHCM certification from the Florida Certification Board within 90 days of hire. Valid driver license with no more than six points on record. Functioning vehicle and automobile insurance (100/300/50) one hundred percent of the time.
