Responsibilities
- Provide RN case management for a home health caseload, performing patient visits and skilled nursing tasks per the plan of care
- Assess patient needs during visits, monitor clinical status, and adjust the plan of care in coordination with the interdisciplinary team
- Complete and maintain OASIS data collection and documentation per regulatory requirements
- Develop, implement, and revise individualized care plans; ensure timely updates and care coordination
- Educate patients and caregivers on disease management, medications, safety, and post-acute care, promoting adherence and safety
- Document all visits and care activities thoroughly; manage a high-volume workload (approximately 5–6 patients per day)
- Provide wound care when indicated and as skilled, adhering to best practices and infection control standards

Requirements
- Active Registered Nurse license in Pennsylvania
- 2+ years of home health experience
- Case management experience and proficiency with OASIS documentation
- Wound care experience preferred
- Ability to perform 5–6 patient visits per day and maintain accurate, timely documentation
- Education: Nursing degree and licensure to practice as an RN
- Willingness to travel within the service area and perform home visits
