Premier Home Health Care is seeking a compassionate, motivated

Physical Therapist Assistant

to join our team full-time. In this role, you’ll provide home-based therapy under the supervision of a Physical Therapist, helping patients work toward their goals in the comfort of their own homes. You’ll be part of a collaborative care team focused on thoughtful, personalized service for patients and families across the Denver Metro area.

What you’ll do

Provide PTA services as ordered and outlined in each patient’s plan of care, under the supervision of a Physical Therapist. Deliver therapy treatments and support patients in carrying out their physical therapy program. Teach patients and caregivers about therapy activities and relevant parts of the plan of care. Observe and document patients’ responses to treatment and report relevant information to the supervising Physical Therapist and care team. Complete accurate, timely clinical documentation. Coordinate services with patients, families, caregivers, and other healthcare professionals. Use therapy equipment and supplies appropriately. Participate in agency training and quality-improvement activities.

What you’ll bring

Current license, registration, or certification as a Physical Therapist Assistant, as applicable in Colorado. Graduation from an approved PTA education program and completion of applicable certification requirements. Current CPR certification and a valid driver’s license. Strong observation, communication, documentation, and problem-solving skills. A collaborative, patient-centered approach to care. Two years of PTA experience is preferred. Community or home-health experience is also preferred.
