Traveling Nurse Practitioner / Physician Assistant – Skilled Nursing Facility Wound Care We are seeking an experienced and independent Nurse Practitioner or Physician Assistant based in the Greater Los Angeles area to provide advanced wound care services within Skilled Nursing Facilities (SNFs) throughout California. As a specialty wound care practice, we focus on delivering evidence-based treatment that improves healing outcomes, reduce avoidable hospitalizations, and support exceptional patient outcomes across post-acute care settings. This facility-based, part-time position is ideal for providers who value autonomy, flexible scheduling, and the opportunity to travel while delivering high-impact clinical care. Providers should be comfortable traveling throughout California based on operational needs, including overnight stays when appropriate. All approved business travel expenses, including airfare, lodging, and transportation, are covered by the company. Minimum availability of 2–3 days per week required. Position Overview Part-time W-2 position Facility-based patient care within Skilled Nursing Facilities Independent, on-site clinical practice Home base in the Greater Los Angeles area Regular travel to Watsonville and surrounding Northern California communities Company-paid airfare, lodging, rental vehicle (or mileage), and approved travel expenses Opportunity to support future Southern California facilities as the practice continues to grow Flexible scheduling based on provider availability Clinical Responsibilities Perform comprehensive wound assessments for SNF residents Develop and manage individualized, evidence-based treatment plans Perform advanced wound care procedures, including debridement and dressing selection Monitor healing progress and adjust treatment plans as clinically indicated Collaborate with Directors of Nursing (DONs), nursing staff, and interdisciplinary care teams Participate in interdisciplinary care planning Educate facility staff on wound care best practices and prevention strategies Maintain timely, accurate, and compliant EMR documentation Support regulatory compliance and quality improvement initiatives Identify at-risk residents and implement preventative interventions Qualifications Required Active, unrestricted California Nurse Practitioner or Physician Assistant license Current DEA registration Current BLS certification Minimum one (1) year of clinical experience Must reside within the Greater Los Angeles area Willingness to travel regularly to Northern California for scheduled patient care Ability to stay overnight as needed during clinical coverage Reliable transportation Preferred Wound care experience Experience with lower extremity or vascular assessment Service Area This position is designed for providers based in the Greater Los Angeles area who are comfortable traveling throughout California to support Skilled Nursing Facilities as operational needs require. Initial assignments are expected to include Northern California, with opportunities to support additional service areas as the practice continues to expand. Providers should be comfortable traveling for scheduled clinical coverage, including overnight stays when appropriate.
