Responsibilities Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function Perform therapeutic exercises and interventions to help clients regain independence and reduce pain Assess and document patient progress, modifying treatment plans as needed Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention Maintain accurate documentation and adhere to all legal, ethical, and professional standards Requirements Licensed Physical Therapist - you must hold a valid license in the state of Michigan Experience - while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
