- Oversee and manage the Case Management department, including the supervision of case managers, scheduling, and daily workflow management.
- Develop and implement strategies to enhance care coordination, discharge planning, and utilization review, ensuring alignment with best practices and regulatory standards.
- Collaborate with interdisciplinary teams to develop individualized care plans, addressing both medical and psychosocial needs to optimize patient outcomes.
- Monitor the department's performance metrics, utilizing data to drive continuous improvement in care coordination and patient throughput.
- Ensure compliance with federal, state, and organizational regulations, including Joint Commission and CMS guidelines.
- A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing (MSN), Healthcare Administration (MHA), or a related field is preferred.
- Current Registered Nurse (RN) license in the state of Texas.
- Minimum of 5 years of case management experience, with at least 2 years in a leadership role.
- Certification in Case Management (CCM) or Accredited Case Manager (ACM) is preferred.
- Strong knowledge of healthcare regulations, utilization review, and care coordination best practices.
