Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Manage a caseload within a defined service territory, primarily covering central and mid-county areas
• Meet productivity expectations of 30 points per 40-hour work week
• Document all patient care and clinical notes accurately using HCHB electronic medical records
• Collaborate with the care team to support patient goals and ensure continuity of care

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of Washington
• Minimum of 1 year of home health experience with a Medicare-certified facility
• Proficiency with HCHB (Homecare Homebase) EMR system
• Ability to meet a productivity standard of 30 points per 40-hour week
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Experience working across varied geographic service territories
• Strong time management and independent practice skills in a field-based setting
