Clinical Assessment Manager — Community Health & Care Planning
VNS Health
VNS Health is seeking a RN to conduct UAS-NY assessments and establish individualized plans of care based on clinical, environmental, and social needs. You will perform face-to-face or telehealth assessments across various communities and coordinate with the care management team to ensure member safety and service adequacy. The role requires a NYS RN license, a Bachelor's in nursing or equivalent, and at least two years of clinical assessment or homecare experience. #J-18808-Ljbffr VNS Health
