Job Description Responsible for carrying out clinical pharmacy services within the Cardiac ICU, promoting clinically and evidence‑based rational drug therapy and sound pharmaceutical care through development of new or expansion of existing pharmacy practice programs, drug therapy policies, and other programs. Serves as a clinical pharmacy resource for the health system and its affiliated pharmacy services. Responsibilities include patient care rounds, clinical interventions, medication use evaluations, committee membership, and acting as a preceptor for residency program and students from local colleges of pharmacy. Conducts research and other scholarly activities throughout the year with the goal of publishing. Supervises and guides pharmacists on optimal provision of care to ensure professional development; directs and disciplines employees as needed. Responsibilities Perform clinical, technical, service, and administrative tasks including departmental activities, policy implementation, and medication safety improvements. Develop and implement policies and procedures to meet professional standards and improve medication safety. Support Pharmacy & Therapeutics committee by preparing drug evaluations and assisting in formulary maintenance. Use an interdisciplinary approach to collaborate with other health‑care professionals to plan, monitor, review, and evaluate drug utilization, safety, effectiveness, and cost. Attend interdisciplinary rounds, provide and follow up on recommended interventions, and document recommendations and interventions. Engage in active patient‑centered medication therapy management, including medication reconciliation, discharge services, patient counseling, comprehensive medication reviews, and targeted interventions. Maintain patient/employee confidentiality in information management. Coordinate the provision of pharmaceutical care by pharmacy department members. Ensure the work environment is well maintained and demonstrate initiative and leadership. Remain current with pharmaceutical databases, national guidelines, and evidence‑based medicine for medication therapy management of disease states. Provide education to patients, family members, and caretakers about disease states and medications, including medication purpose, administration, monitoring, and adherence. Apply knowledge and skills to provide care appropriate to the age of patients served, modifying approaches based on patient age‑specific needs. Participate in the development of other staff members by identifying learning needs and delivering education on medication appropriateness, dose adjustments, drug selection, interactions, cost, and patient safety. Serve as preceptor for residency program and students from local colleges of pharmacy. Design and implement training and development programs in a timely fashion. Supervise pharmacy employees, interview candidates, implement discipline and corrective action plans, promote employees based on assessment of clinical skills, perform employee performance evaluations, attend management meetings, and provide off‑hour coverage. Qualifications Doctor of Pharmacy Degree (required) PGY‑2 in Critical Care Board Certification (e.g., BCPS, BCCCP) strongly encouraged 5 years hospital experience or equivalent, with clinical experience/training. Licensed Registered Pharmacist issued by NYS Board of Pharmacy. Continues to meet regulatory, educational, and licensure requirements throughout employment, as set forth by New York State regulations. Achieves annual requirement of 15 continuing education hours (ACPE approved).
