Job Details Job Location: HUGO, CO 80821 Salary Range: $32.00 - $38.00 Hourly Job Shift: Any Purpose The Registered Nurse plans and delivers nursing care to patients/residents in Home Health and Hospice. Dimensions The Registered Nurse works under the supervision of the LCHH Director or in the absence of the LCHH Director, or alternative director of LCHH. Essential Duties and Responsibilities Maintains current professional credentials. Is committed to personal and professional growth. Seeks opportunities for self-growth in knowledge and skills within the home health agency and nursing profession. Projects professional image; knows and follows dress code, including wearing nametag. Demonstrates good personal hygiene and presents a professional appearance. Utilizes time in an appropriate and effective manner. Notifies office at least two (2) hours in advance if unable to work, and twenty-four (24) hours in advance for non-emergent situations. Vacation requests are due as far in advance as possible. Communicates effectively, in a professional and respectful manner with other members of the home health staff, hospital, patients and families. Efficiently works with peers and with a team approach. Adapts to changing standards and practices in a positive manner. Knows and follows OSHA procedures. Practices proper lifting techniques. Follows policies and procedures for infection control and facility safety in accordance with regulatory guidelines. Maintains CPR certification. Attends 70% of staff meetings/case management. Collects venipunctures and reports results to MD in a timely manner (usually same day). Displays honest and good work ethics. Maintains communications with director and staff. Participates in QI/QA program as requested by Director. Performs managerial duties as requested by Director. Participates in the patient’s medical, emotional, psychosocial, spiritual, safety, and financial needs. Contributes to the Plan of Care (POC), reevaluates and makes revisions when necessary. Follows established physician orders/POC and administers patient care accordingly. Initiates appropriate preventive and rehabilitative services, i.e., PT, OT, ST, MSW. Demonstrates behaviors that promote positive customer relations with physicians, staff, patients and visitors. Recognizes and provides patient/family/caregiver education in a clear, concise, easily understood method. To include verbal and written instructions. Promotes self-sufficiency and records outcome of instructions and teaching. Demonstrates practices to keep all patient/medical record information protected and confidential. Seeks supervisory direction/counsel when appropriate. Shares learning experiences and teaches others, to include RN, LPN, CNA, PT, OT, ST, MSW, as indicated. Demonstrates behaviors consistent with facility mission, vision and values. Accepts assigned duties in a cooperative manner. Adheres to hospital wide policies and procedures within the department as outlined in the employee handbook. Accurately follows written and verbal directions. Performs other related duties as assigned or requested. Uses time and effort of all staff so that self and others work at an appropriate level and volume. Completes SOC and 485 in a timely manner. (24-48 hours). Supervises, teaches and orients other nursing personnel. Utilizes the Agencies resources in a cost efficient and effective manner. Follows chain of command with complaints, suggestions, and comments. Insures that the patient’s right to privacy and dignity is maintained and protected at all times. Follows patient’s rights at all times. Participates in educational programs and in-service meetings. Completes at least 20 hours of continuing education credits a year either through topics assigned or appropriately selected to meet regulation. Completes daily visit notes in a timely manner. (24-48hrs.) Able to improvise and respond appropriately to problems in the home setting. Clinical notes, recording of reports are written within 24-48 hours. Discharge Summaries/Oasis is due within 3 days of discharge/transfer/death. All work is to be completed prior to vacation. Agency and patient paperwork is recorded in a clear, concise, organized, correct format. Carries out assigned tasks with minimal supervision, and performs all duties in an effective manner. Reports to work as scheduled and consistently demonstrate dependability and punctuality. Sets up home visits in an organized and timely manner so that mileage and time are kept to the minimum. Reports the need for overtime to the Home Health Director for pre-approval. Provides Home Health Aide with Plan of Care. Reports progress and effectiveness of care to referring physicians or agency. Demonstrates skill, proficiency, and safety in administering patient care. Assumes responsibility for safe, economic use of equipment and supplies. Qualification Requirements To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience Registered Nurses must be currently licensed and in good standing in the State of Colorado. For expanded functions, (i.e., chemotherapy, psychiatric, specialized infusion, etc.) completion of an organized educational program in a specific program area is required. May include recent pertinent continuing education or certification. Prefer one (1) year experience home health, including complex patient caseload. Minimum of two years general nursing in an acute care setting, plus one (1) year home health; but will take maturity level, education, and life experience into account on an individual basis. Other Skills and Abilities Registered Nurses must have thorough knowledge and skill in current practice and will be responsible for notifying the Home Health and Hospice Director of any procedures unfamiliar with and will request in servicing and/or direct supervision with performance of procedure. Physical Demands Walking, standing, reaching, bending, lifting (50lbs), grasping, pushing and pulling, fine hand coordination. Ability to communicate clearly and concisely with patients, families, and personnel. Ability to distinguish smells and temperatures. Ability to hear and respond to pages. Ability to read and write in English. Ability to remain calm under stress.
