PRN Hospice Social Worker

What You Will Do As a PRN Social Worker Complete comprehensive psychosocial assessments and develop individualized care plans based on patient and family needs Provide counseling, emotional support, crisis intervention, and education to patients and caregivers Serve as a patient advocate while coordinating care with medical providers, community agencies, and the interdisciplinary team Assist patients and families with advance care planning, financial and community resources, and end-of-life decision making Facilitate family meetings and collaborate with chaplain, bereavement, nursing, and other team members to provide holistic care Maintain accurate and timely documentation in the electronic medical record What We Are Looking For Master's degree in Social Work from an accredited program required (or a related Master's degree with qualifying healthcare experience) Two years of case management or counseling experience, including at least one year in a healthcare setting preferred Strong assessment, counseling, crisis intervention, case management, and communication skills Ability to work independently while collaborating effectively within an interdisciplinary team Comfort with EMR and technology Reliable transportation with a valid driver's license and insurance A heart for mission-driven, relational work with patients and families Schedule PRN - we are looking for a candidate that can be available to cover about 1 shift a week.
