Pay Range: 2100-2300
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler or local candidated accepted
Requirements: 1 year of Medicare Certified Home Health experience. 1 year of HCHB EMR and OASIS experience
Responsibilities:

• Provide comprehensive nursing care to home health patients.
• Assess patient needs and develop individualized care plans.
• Monitor patient progress and update healthcare team accordingly.
• Educate patients and their families on health management and disease prevention.
• Document patient care activities accurately and timely.
