Hospice RN Case Manager

Role Overview Provides specialized nursing care and support to patients at the end of life, coordinating comprehensive care plans in collaboration with an interprofessional team, patients, and families. Responsibilities include assessing patient needs, developing and implementing individualized care plans, promoting cultural sensitivity and patient-centered care, evaluating patient condition changes, and ensuring communication across the continuum of care. This safety-sensitive position requires maintaining a high level of alertness, with duties performed primarily Monday through Friday, including on-call and holiday rotations. Qualifications include a Nursing Diploma or Associate's Degree with RN licensure, at least 2 years of experience, and a valid driver’s license. Preferred qualifications include a Bachelor's Degree in Nursing and 2-5 years of experience in home health or hospice care. Key skills involve patient assessment, care planning, cultural competency, critical thinking, collaboration, and evidence-based practice.
