Hospice/Home Care Register Nurse

In this role, you will support patients and their families through end-of-life care and chronic illness management, ensuring dignity, comfort, and quality of life. Responsibilities Provide skilled nursing care to hospice and home care patients according to established plans. Assess patient condition and effectively communicate changes to the care team. Develop, implement, and update individualized care plans in collaboration with interdisciplinary teams. Educate patients and family members regarding illness, treatment options, and self-care techniques. Administer medications and treatments as prescribed by physicians. Document all patient interactions and care activities accurately and timely. Coordinate with physicians, social workers, and other healthcare professionals to provide holistic care. Requirements Current Registered Nurse (RN) license in the state of Michigan. Experience in hospice or home care nursing preferred. Strong assessment, communication, and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Valid driver’s license and reliable transportation. Compassionate attitude with a commitment to patient-centered care.
