Physical Therapist - Outpatient Setting

Job Description

We are seeking a dedicated and experienced Physical Therapist to join our outpatient team. This position will focus on providing therapy evaluations and treatments for a patient caseload that includes adolescents to geriatric patients. The therapist will engage in managing cases across orthopedic, neurological, and general medical conditions, ensuring adherence to established policies and procedures.

Responsibilities

• Conduct patient assessments and develop individualized treatment plans.
• Provide therapy interventions and monitor patient progress.
• Collaborate with healthcare team members to optimize patient care.
• Maintain accurate and timely documentation of patient care activities.

Schedule

This is a full-time position requiring 40 hours per week across 4-5 days. Occasional weekend and holiday coverage may be necessary.

Location

Position requires a permanent address of 50+ miles from the facility. (specific city and state not provided).

Required Experience / Certifications / Licensure

• Minimum of 2 years of current Physical Therapy experience (required).
• Outpatient experience (required).
• Inpatient experience (highly preferred).
• Active American Heart Association BLS certification (required).
• Current and valid state Physical Therapy license (required).
• Profile requirements including physical therapy skills checklist and a supervisory signed reference from the past year of employment.
