- Provide skilled nursing care to patients, including wound care, medication administration, and treatment procedures
- Assess patients' physical and emotional needs and develop individualized care plans to address them
- Collaborate with our interdisciplinary team to ensure seamless transitions and coordinate patient care
- Educate patients and their families on treatment plans, medications, and self-care techniques
- Document patient progress and maintain accurate records

- Current LPN/RN licensure in the State of Louisiana
