Registered Nurse - Senior Care Coordinator - Full-Time - Day Shift

Role The Care Coordinator RN Sr. assesses patient needs, coordinates care across the continuum, and supports discharge planning for complex cases during hospitalization. Responsibilities Conducts psychosocial and clinical assessments Collaborates with physicians and care teams Manages discharge planning Ensures timely care progression Acts as a resource for team members Mentors new staff Participates in interdisciplinary rounds and patient education Documents care activities accurately Identifies barriers to discharge Facilitates post-acute referrals Qualifications Requires an Associate's or Bachelor's in Nursing RN licensure (single state or multi-state) BLS certification Minimum of 3 years' acute care experience plus 2 years in hospital-based case management Skills Excellent communication Strong assessment and organizational skills Ability to handle complex situations Knowledge of healthcare regulations Community resources Must be self-directed, mature, and collaborative Overall Overall, the role focuses on delivering patient-centered care, optimizing resource utilization, and ensuring effective transition from hospital to next care setting. #J-18808-Ljbffr MEP Health
