Work Environment

The successful candidate will work onsite, primarily within our home care setting, allowing us to provide personalized therapy directly in the patient's home. Key Responsibilities

Your day-to-day responsibilities will include:

• Conducting thorough evaluations of patients' physical capabilities to determine tailored treatment plans.
• Administering various therapeutic modalities, including manual therapy, exercise programs, and pain management techniques.
• Monitoring and documenting patient progress in a systematic manner, utilizing electronic health records (EHR) for accurate reporting and adjustments to care plans.
• Encouraging and educating patients and their families on home exercise programs and strategies to maintain physical well-being beyond therapy sessions.
• Participating in interdisciplinary team meetings to collaborate on comprehensive care strategies, addressing not only physical health but also emotional and social factors affecting recovery.
• Maintaining compliance with regulatory standards and best practices to ensure the highest quality of care.

Patient Population / Client Focus

You will primarily work with orthopedic patients, including those recovering from surgeries, trauma, and chronic pain conditions. Your expertise will be crucial in assisting patients to regain strength, mobility, and functional independence, thereby enabling them to return to their desired lifestyles. A special emphasis will be placed on geriatric populations, as many of our patients require focused care aligned with the complexities of aging.

Qualifications

The ideal candidate will possess:

• A valid Physical Therapist license in the state of Florida.
• A Bachelor's Degree in Physical Therapy; however, a Master's Degree or Doctorate in Physical Therapy from an accredited institution is preferred.
• Strong communication, organizational, and interpersonal skills to effectively engage with patients and collaborate with team members.
• A commitment to patient advocacy and delivering exceptional therapy services in a collaborative environment.

Preferred Experience

While a minimum of two years of experience as a physical therapist is required, prior experience in skilled nursing facilities or working specifically with geriatric patients is highly desirable. Familiarity with home care setting challenges and opportunities will be beneficial in this role.
