Job Description

As an RN Case Manager at Visiting Nurse and Hospice for Vermont and New Hampshire, you will interact closely with patients, becoming an integral part of their care team. Enjoy a flexible work/life balance while providing exceptional nursing, rehabilitation, hospice, and personal care services.

Responsibilities

• Develop care plans in collaboration with the patient's physician, the patient, and their caregivers upon admission.
• Periodically review and recertify the patient care plan as necessary.
• Coordinate the care plan through interdisciplinary communication and case conferences.
• Advise other disciplines of discharges in a timely manner.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status.
• Document evaluations, treatment goals, and objectives accurately.
• Teach self-care techniques to patients and caregivers.
• Provide medication, diet, and other instructions as ordered by the physician.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• 2 years of experience in Home Health.
