Licensed Practical Nurse

Community Health Programs (CHP) is actively seeking a dedicated Licensed Practical Nurse (LPN) to join our growing network of health centers. At CHP, we prioritize a holistic approach to health by integrating medical, dental, and behavioral services to fully support the wellness of the communities in Berkshire County. As an LPN at CHP, you will play a critical role in delivering high-quality, community-based care in a supportive and collaborative environment. Responsibilities Administer nursing care to ill, injured, or disabled patients within the guidelines set by the facility. Monitor patient health and maintain accurate records of vital signs and patients' health progress. Assist in the implementation of patient care plans set forth by registered nurses or doctors. Prepare and administer injections and prescribed medications as directed by the physician. Perform routine lab tests and therapeutic services. Educate patients and their families on health maintenance and disease prevention. Collaborate with a multidisciplinary team of healthcare providers to enhance patient care. Qualifications Proficient in administering medications and immunizations. Strong communication and interpersonal skills. In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.). Excellent organizational and time management skills. Ability to make decisions quickly and in high-pressure situations. Valid LPN license to practice in the state of Massachusetts is required.
