Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Warr Acres in Warr Acres, Oklahoma, United States. As a dedicated healthcare provider, you will provide high-quality patient care and collaborate with our interdisciplinary team to deliver exceptional results. Licensed Practical Nurse (LPN) is a full-time position that offers the opportunity to make a meaningful difference in the lives of our patients.

Responsibilities:

• Provide direct patient care to a diverse patient population, including assessing, planning, implementing, and evaluating patient care plans

• Collaborate with the interdisciplinary team to develop and implement individualized patient care plans

• Administer medications, treatments, and therapies as ordered by physicians

• Conduct vital signs, monitor patient conditions, and report changes to the healthcare team

• Maintain accurate and confidential patient records and reports

• Participate in patient education, counseling, and discharge planning

Requirements:

• Current LPN licensure in the state of Oklahoma

• 1+ years of experience in a long-term care or acute care setting

• Ability to work effectively in a fast-paced environment with minimal supervision

• Strong communication and interpersonal skills

• Ability to lift, push, and pull a minimum of 50 pounds

• CPR certification and TB test results
