The PTA Physical Therapist Assistant plays a crucial role in delivering home health care services to geriatric patients. This position involves working closely with a Physical Therapist within a multi-disciplinary team to provide therapeutic exercises, patient assessments, and ongoing care in patient homes. The PTA is responsible for ensuring safety compliance and documenting patient progress while frequently traveling to patient locations.

Responsibilities

• Deliver patient care and therapeutic exercises in home health settings
• Perform patient assessments to tailor treatment plans
• Document patient progress and clinical notes accurately
• Educate patients and families on rehabilitation techniques
• Collaborate with Physical Therapists and multi-disciplinary team members

Required Qualifications
