Nurse PractitionerTryfacta is seeking a Nurse Practitioner for our client in Malone, NY 12953. This is a 2-year contract assignment.Position Title: Nurse PractitionerLocation: Malone, NY 12953Duration: 2 yearsWork Schedule: Monday-Friday (day shift)Responsibilities for this position include, but are not limited to:Provide comprehensive medical evaluations and ongoing care for incarcerated individuals with chronic illnesses, physical disabilities, and complex healthcare needs.Assess, initiate, and monitor Medication-Assisted Treatment (MAT) programs, including patient evaluations, treatment planning, and follow-up care.Deliver medical services within the facility's 18-bed infirmary, managing admissions, discharges, and high-volume patient turnover.Document assessments, treatment plans, orders, and clinical encounters accurately in the electronic medical record system.Collaborate with correctional staff, nursing personnel, and multidisciplinary healthcare teams to ensure continuity of care and timely medical interventions.To be considered for this position, you should have:Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license, with all required certifications and prescriptive authority as applicable.Experience providing medical care to patients with chronic diseases, disabilities, behavioral health conditions, and complex healthcare needs.Knowledge of Medication-Assisted Treatment (MAT) programs, substance use disorder management, and patient evaluation protocols.Experience working in correctional healthcare, infirmary settings, emergency care, urgent care, or other high-volume clinical environments preferred.Strong clinical assessment, documentation, communication, and interdisciplinary care coordination skills, with proficiency in electronic medical records (EMR) systems.Tryfacta is an Equal Opportunity/Affirmative Action Employer.
