Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Milford, New Hampshire. Start date is August 31, 2026, for a 13-week contract. Responsibilities
- Conduct initial evaluations and ongoing assessments to determine PT needs for residents.
- Develop and implement individualized treatment plans, including gait, balance, strength, and mobility training.
- Administer therapeutic interventions and monitor patient response; adjust plans as needed.
- Document evaluations, progress notes, and discharge plans to support regulatory and care-team requirements.
- Collaborate with an interdisciplinary team (nursing, OT, speech therapy, social work) to optimize resident outcomes.
- Educate residents and families on safe-occupational strategies, home exercise programs, and fall prevention.

Requirements
- Active physical therapy license in New Hampshire (or eligibility to practice in NH)
- Graduation from an accredited physical therapy program (DPT preferred)
- Current Basic Life Support (BLS) certification
- Experience or strong interest in skilled nursing facility or geriatric rehabilitation
- Strong communication skills and ability to work in an interdisciplinary team
- Able to work on-site in Milford, NH and participate in a 35-hour weekly schedule
