Description: The Case Manager is responsible for medical necessity review, discharge planning, and utilization review. This role identifies and communicates changes in the patient's plan of care to the healthcare team. The Case Manager maintains detailed knowledge of community resources and the process for referrals. Using length of stay metrics, the Case Manager anticipates patient needs while ensuring stewardship of resources. Timely transitioning of patients to an alternative level of care is essential. Documentation in the patient record must be consistent and timely. This includes initial screenings and discharge plans. First-time travelers are accepted for this position.
