Job Title: Care Manager Profession: Care Manager Specialty: ED Duration: 8 months Shift: Not specified Hours per Shift: 35 hours Experience: Minimum of five years of clinical experience as a Registered Nurse; leadership experience preferred License: RN Licensed as a Registered Nurse Certifications: Baccalaureate degree required; PRI and Screen certification within three months of employment is required; Master’s Degree and Bilingual Spanish Certification in Case Management preferred Must-Have: - Hospital experience - Bilingual (Spanish speaking) - Care coordination experience - Experience reviewing assessments Description: The Care Manager develops, facilitates, monitors, and communicates a care plan in partnership with the member, their family or significant other, and primary caregiver. The Care Manager works with the primary and/or attending physicians and various providers. Using assessments and interviews conducted by the Assessment Nurse, the Care Manager identifies risk factors, strengths, challenges, and service needs of the member to help them stay safe and well in their community setting. The Care Manager reviews and evaluates assessment and information for members in managed long-term care services. They develop a working relationship with the primary care provider (PCP) to discuss the care of the member. The Care Manager reviews assessment findings with the PCP to identify concerns not raised by the clinical team. They assign risk categories to the members based on identified risk factors. The Care Manager, as part of the Care Management team, develops a formal care plan for all required services for the member, including a disaster plan. They monitor the condition of all members at least monthly, typically by telephone, but face-to-face when necessary. The Care Manager identifies clinical issues needing immediate assessment or treatment to reduce the risk of unnecessary hospitalizations, emergency department visits, or nursing home admissions. They also identify opportunities to improve care quality by ensuring members receive necessary preventive and chronic disease care. The Care Manager approves requests for additional services based on assessments and using evidence-based standards and refers denials or limitations to the Medical Director. They assist members in coordinating services both within and outside networks as necessary, including facilitating discharge from acute settings. The role involves care coordination throughout the continuum of care. The Care Manager optimizes both the quality of care and quality of life for members. They coordinate with the Utilization Management (UM) department on concurrent and retrospective reviews. The Care Manager follows up with assigned nurses for clinical updates to the care plan. Documentation of coordination notes and routine contacts with members is completed within two business days according to the assigned risk level. Participation in team care planning meetings is expected. The Care Manager handles complaints that can be resolved in one day. They assist customer service and the UM department by providing necessary records and materials for grievances from members. Communication with members who are delinquent in payments is also part of the role. Collaboration with all departments is essential. The Care Manager identifies members suitable for specialty programs. They are responsible for performing all managed long-term care activities in compliance with regulatory agency requirements. Providing information on all requests from the Quality Management Department for review by various Quality committees is required. Additional tasks assigned by department leadership are expected to be completed. Participation in the department’s on-call schedule is also required, as it is rotated among staff.
