Job Title: Clinical Nurse Profession: RN Specialty: Stepdown Duration: 13 weeks Shift: 7PM - 7:30AM Hours per Shift: 12.5 Experience: Minimum 2 years RN experience, 2 years of cardiology, tele, VAD experience License: NYS RN License Certifications: BLS Must-Have: BSN, Ventilator experience, Stepdown experience Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of professional practice and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care while delegating to others as appropriate. Educates patients and caregivers while anticipating needs and readiness to learn about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family, and all members of the interdisciplinary care team. Manages assignments within the care delivery model and demonstrates effective allocation of material resources. Role models the principles of relationship-centered care. Considers patients' values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to increase understanding of patients' perceptions of care. Initiates service recovery for patients and family members who have concerns. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions. Conducts hourly rounding on assigned patients addressing essential needs. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on evidence-based procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research studies. Contributes to quality and safety practices and compliance measures. Escalates potential safety hazards or gaps from best practice as needed. Utilizes information technology by documenting nursing practice to support quality initiatives. Performs charge nurse duties as assigned. Coordinates with clinical leadership to ensure appropriate utilization of supplies and inventory for patient care. Escalates resource deficits to appropriate leadership. Documents accurately and thoroughly in compliance with regulatory and legal requirements. Facilitates patient throughput efficiently. Contributes to the environment of care to support colleagues, patients, and their families. Advances clinical competence to progress from novice to expert. Projects a professional image and communicates effectively with compassion. Incorporates ethical principles into decision-making for patients and families.
