Clinical Practice Preferred Coordinator

Clinical Practice Preferred Coordinator Location: 5531 South Hulen Street, Fort Worth, TX (on‑site). Full‑time position (40 hours per week) Monday through Friday, 8:00 am to 5:00 pm CST with flexibility for occasional overtime. Primary Responsibilities: Partner with WellMed/USMD physicians to assist patients in completing health screens and preventive care measures that are not yet completed. Outreach to external physician offices to obtain copies of consult notes and reports for patients who had preventive healthcare screens and clinical measures completed at outside facilities. Update preventive health screens and clinical measures in the EMR that have been determined as complete. Communicate regularly with interdisciplinary team members to coordinate and update a patient’s care. Maintain an accurate tracking log of patients who have been outreached and the status of their preventive health screens and clinical measures. Participate in various quality‑driven initiatives within WellMed/USMD and collaborate to achieve high‑level patient outcomes. Submit data to external agencies as directed by management. Explain the importance of preventive health screens and clinical measures to patients and families. Perform all duties within the scope of a Medical Assistant, including operating diagnostic equipment, obtaining cultures, administering non‑intravenous medication, performing specimen collection, and performing EKGs. Perform quality control checks on equipment and prepare and sterilize medical equipment using the autoclave. Foster accountability for clinical practice and patient goals. Record patient care documentation accurately and in a timely manner, and assist other MAs with documentation questions. Coordinate patient care as directed by providers, policies, and procedures. Respect patient confidentiality at all times and ensure others also do so. Organize exam and treatment rooms, stock and clean rooms, and sterilize instruments. Create and maintain an environment that supports infection prevention. Conduct telephone and in‑person screening limited to intake and gathering of information without requiring judgment based on clinical knowledge. Rotate clinic to clinic within WellMed/USMD clinic locations as needed. Perform any other related duties as assigned. Required Qualifications: High School Diploma / GED or equivalent work experience. Must be 18 years of age or older. Current BLS certification. 3+ years as a certified Medical Assistant or a combination of 5 years certified and non‑certified medical assistant experience (with at least a minimum of 2 years certified). Experience with computers and literacy in electronic health records. Experience working with Microsoft Office, with a focus on Excel (view, sort, filter, create spreadsheets, VLOOKUP). Knowledge of medical terminology. Availability to work between 8:00 AM and 5:00 PM CST from Monday to Friday. Preferred Qualifications: Bilingual fluency in English and Spanish. Soft Skills: Ability to mentor others. Ability to react calmly and effectively in emergency situations, assuming a leadership role until higher levels of care arrive on scene. Strong communication and customer service skills. Physical and Work Environment: Ability to sit for extended periods of time. Ability to stand for extended periods of time. Ability to use fine motor skills to operate office equipment. Ability to receive and comprehend instructions verbally and/or in writing. Ability to use logical reasoning for simple and complex problem solving. Benefits and Compensation: Hourly pay ranges from $20.38 to $36.44 per hour based on full‑time employment.
