Registered Nurse Case ManagerWe are seeking a dedicated and compassionate full-time Registered Nurse Case Manager to join our healthcare team. In this rewarding role, you will coordinate and oversee the individualized care of patients, ensuring they receive the highest standard of medical services in their home environment. This position offers the opportunity to make a meaningful impact in the lives of patients and their families while working within a supportive interdisciplinary team.Schedule: Full-Time, Weekdays, On-Call (not required, but appreciated), Day ShiftKey Responsibilities:Coordinate and manage the comprehensive care of assigned patients.Develop, implement, and regularly update individualized care plans.Perform skilled nursing assessments and provide direct patient care.Collaborate with physicians, therapists, social workers, and other healthcare professionals.Educate patients and families on disease processes, medications, treatments, and care plans.Ensure timely and accurate documentation of all care activities.Participate in case conferences and interdisciplinary team meetings.Serve as a compassionate resource for patients and families, providing emotional support and guidance.At Advanced Home Health and Hospice of Lehigh Valley, we are committed to delivering compassionate, high-quality care while building strong, meaningful relationships with our patients, families, and communities.
