Social Worker
Job Summary:
Woodbury Heights Nursing and Rehabilitation Center, a compassionate and caring healthcare facility, is seeking a dedicated Social Worker to join our team in Woodbury, NY. As a Social Worker at Woodbury Heights, you will play a vital role in providing personalized support to our residents, ensuring their emotional and social well-being. Responsibilities:
Assess the social and emotional needs of our residents and develop personalized care plans to address their concerns
Provide counseling, guidance, and support to residents and their families to ensure their overall well-being
Collaborate with other healthcare professionals to ensure a holistic approach to care
Develop and implement activities and programs to promote socialization, engagement, and mental stimulation among residents
Maintain accurate and detailed records of resident interactions and care plans
Participate in interdisciplinary team meetings to ensure collaborative care
Provide referrals to external resources and services as needed
Requirements:
Bachelor's degree in Social Work or related field
Valid license to practice as a Social Worker
0 years of experience (entry-level candidates welcome)
Strong communication and interpersonal skills
Ability to work with diverse populations and adapt to changing priorities
Familiarity with healthcare regulations and compliance standards
