What You'll Do

• Provide, teach, and document nursing care to individuals and families at home
• Promote, maintain, and restore health through independent interventions
• Collaborate with healthcare providers and community resources to coordinate care
• Assess patient responses to treatment and adjust care plans accordingly
• Exercise independent judgment in emergency situations
• Supervise others when required and maintain confidentiality
• Utilize effective time management to meet patient needs
• Follow all protocols, guidelines, and policies specific to the community health setting

What You Need

• Valid Massachusetts RN license (must include license upload with profile)
• Minimum 1 year of community health or home health nursing experience
• BLS and DEA certification
• Ability to exercise independent judgment
• Strong interpersonal and communication skills
• Valid driver's license and ability to drive in all weather conditions
• Willingness to accept on-call responsibilities and flexible work hours

• Experience with home health care and patient education
• Ability to work independently with minimal supervision
