Job Overview Primary Care Nurse Practitioner / Physician Assistant position located in the Boston, Massachusetts area. The role is permanent and supports a Medicare‑focused patient population (65+). Responsibilities Provide primary care for 15–18 patients per day on a hybrid schedule: four onsite days (8:00AM–5:00PM) and one remote day each week. Maintain a light call schedule (≈ two evenings per month and approx. two weekends per year). Coordinate care with specialists and hospitals; use Athena EMR. Deliver comprehensive, team‑based primary care focused on chronic disease management, preventive care, and social support. Address social determinants of health including housing, food security, and social isolation. Collaborate with physicians and APPs in a team‑based setting. Qualifications Board certified or truly board eligible Nurse Practitioner or Physician Assistant. Minimum of two years of post‑training primary care experience. No visa sponsorship available. Benefits Base salary: $140,000–$160,000 DOE. 10% bonus opportunity.
