Job Details:

Schedule: Monday-Friday, 8-hour shifts between the hours of 7:00 AM and 6:00 PM

Guaranteed Hours: 40 hours per week

On-Call Requirement: At least 1 night per week and 2 nights during one week of the schedule rotation, plus weekend on-call coverage

Location: San Marcos, TX

Job Type: Contract

Pay Range: $52-$54/hour *Pay offered is based on experience, expertise, credentialing, and education.

Duties

Provide comprehensive hospice nursing care to patients and families.

Perform patient admissions, routine visits, and end-of-life care visits.

Manage and coordinate patient care plans in collaboration with the interdisciplinary team.

Complete deaths visits and provide appropriate documentation and support.

Complete accurate and timely documentation in accordance with agency standards.

Participate in required on-call rotations, including nights and weekends.

Requirements

Active RN License

Hospice experience required

Recent HCHB (Homecare Homebase) experience required

Ability to perform admissions, deaths, GIP care, and routine patient visits independently
