Primary Care Physician Physician

Join a Leading Primary Care Team. We are seeking a Primary Care Physician (PCP) to lead patient-centered, outpatient care within a multidisciplinary team environment focused on quality and value-based care. Key Responsibilities Provide comprehensive outpatient medical care, including health maintenance and disease prevention. Maintain accurate medical histories and records, referring patients to specialists as needed. Engage in team-based care models, collaborating with various healthcare professionals to optimize patient outcomes. Make clinical decisions on moderately complex issues, exercising autonomy in approach and objectives. Qualifications and Skills Current, unrestricted medical license; eligible for licensing in the practice region. Graduate of an accredited MD or DO program. Board Certified or Board Eligible in Family Medicine or Internal Medicine (Geriatric Medicine preferred). Active DEA license and excellent communication skills. Strong interpersonal skills, adaptability to value-based care, and proficiency with EMR and computer tools. Willingness to participate in TB screening programs. Preferred Experience Two to five years of relevant clinical experience. Experience managing Medicare Advantage panels and understanding quality metrics like HEDIS.
