What You'll Do

• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Follow-Up, Supervisory, and Discharge visits.
• Perform comprehensive patient assessments and develop individualized Plans of Care.
• Develop and manage CMS-485 Plans of Care and physician orders.
• Review clinical documentation for quality, accuracy, and regulatory compliance.
• Mentor and support RNs, LPNs, and other clinical staff.
• Conduct clinical competencies, field training, and new nurse orientation.
• Provide real-time clinical guidance and problem-solving support.
• Collaborate with physicians, case managers, caregivers, pharmacies, and interdisciplinary team members.
• Participate in Quality Assurance and Performance Improvement initiatives.
• Promote compliance with Medicare, Virginia DMAS, and agency policies.
• Serve as a clinical leader who helps strengthen our culture of excellence.

Qualifications

We're looking for an RN who is confident, clinically strong, and passionate about mentoring others.

Required

• Active Virginia RN license
• Minimum five years of recent Home Health experience
• Experience independently completing SOC, ROC, Recertification, Follow-Up, and Discharge visits
• Experience with OASIS documentation
• Experience developing CMS-485 Plans of Care
• Strong knowledge of Medicare Conditions of Participation and Virginia DMAS regulations
• Previous leadership, preceptor, or supervisory experience
• Excellent communication and critical-thinking skills
• Valid driver's license, reliable transportation, and auto insurance
