Breakthrough is seeking a dedicated Case Manager for the Back 2 Our Future (B2OF) program in Chicago, IL. You will provide culturally relevant case management, care coordination, and supportive services to youths aged 13-21 under QMHP supervision. Responsibilities include intake, goal setting, group facilitation, service coordination, and documentation.
