POSITION SUMMARY
The Case Manager, under the supervision of a Qualified Mental Health Professional, will work collaboratively with the Breakthrough Beyond team, Violence Prevention team and Behavioral Health team to provide culturally relevant case management, care coordination and support services to youth ages 13-21 to assist youth with achieving their wellness goals. The incumbent accepts the responsibilities and fulfills the obligations as outlined in the position description.ESSENTIAL RESPONSIBILITIES / FUNCTIONS
Identifies and develops relationships with at risk individuals within target community Develop and maintain supportive and effective relationships with participants Communicate, and coordinate with team members, the team lead, and authorizing LPHA, as well as other service partner(s) all relevant information as Releases of Information allow Complete and coordinate intake process Set and monitor goals related to safety and case management Develop, deliver, and document one-on-one support sessions and facilitate participant groups Assist participants with transportation to community supports, court, social service agencies, etc. Coordinates internal and external services being provided to participant; identifies and arranges resources Assist with the facilitation of community support and skill-building groups Create coordinated care through regular communication with the team, participant family, and other service providers Assist in development of coping skills Develops partnerships with resources that will strengthen support for participants and hold partners accountable as needed Maintains participants' records by inputting case notes, assessments, and events daily in online data system Engage in comprehensive care coordination to link youth to community-based support to promote ongoing improved wellness Prepares participants' exit strategy by reviewing goals and individualized plans, coordinating discharge and post-discharge requirements, orienting and training family members, and providing resources Monitors cases by verifying participants' attendance; observing and evaluating activities and responses; advocating for needed services; identifying and connecting to additional resources; intervening in crises; and providing personal support Communicates participants' progress by engaging in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to team and family, and identifying program influences. Maintains current knowledge regarding services to adolescents, adults, and families. Participates in agency in-service trainings as well as self and supervisor directed advanced skill-trainings. Maintains and adheres to current rules and regulations pertaining to services delivered by the Department of Human Services, Department of Children & Family Services, Illinois Healthcare & Family Services, including the Abused and Neglected Child Reporting Act (ANCRA), and the Office of the Inspector General. Supports Behavioral Health integration of partnerships, groups and initiatives to ensure that all youth are linked with appropriate treatment, educational, social, recreational, and medical resources during and prior to completion of service. Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field OR Bachelor's degree in any field, with two (2) years of documented clinical experience in a mental health setting under the supervision of a QMHP OR High school diploma or GED and a minimum of five (5) years documented clinical experience in mental health or human services Experience supporting and working with high-risk populations, in the field of Violence Prevention Experience providing comprehensive trauma-informed case management Demonstrated track record of accountability Excellent relationship-building skills as well as customer service skills Ability to work with a team, respond well to supervisor direction, be flexible and dependable, and demonstrate initiative Ability to demonstrate sensitivity to the service population’s cultural and socioeconomic characteristics Demonstrates comfort and skill using basic computer software such as Microsoft Word, Excel and Outlook as well as other Windows based programs Must maintain a valid driver’s license, have daily access to a vehicle and an acceptable motor vehicle driving record and/or reliable means of transportation are required Required Knowledge, Skills and Abilities Communicate effectively, both orally and in writing. Interpret and explain policies, procedures, and relevant laws Working knowledge of the complex needs of returning citizens and individuals affected by gun violence Knowledge of the community and street life/organizations Knowledge of/experience working with individuals who have experienced or suffered from trauma, PTSD and or anxiety Understanding of substance use disorders and effective approaches to care. Understanding of client confidentiality and adherence to their professional code of ethics. Ability to demonstrate strong interpersonal skills, professionalism, and problem solving ability. Ability to engage youth at all stages of change. Ability to develop and retain relationships with other service providers Crisis intervention training or knowledge Knowledge of Chicago and communities with histories of violence Ability and commitment to maintain high level of confidentiality Ability to understand and follow safety/security practices and work in high pressure situations Must have the ability to use initiative, discretion and independent judgment in making decisions. Ability to demonstrate initiative and follow through and be comfortable working in a changing environment. Demonstrates willingness to consistently improve work performance through their relationship and guidance. Must possess an accurate and informed understanding of cultural and urban realities, especially as related to youth, families, poverty, and community development.SUPERVISION

Vacation Days -Full-time employees in this position are eligible to accrue vacation days at a rate of 6.67 – 13.33 days per month based on tenure with the organization. Personal Days -Full-time employees in this position are eligible for up to 3 personal days annually, pro-rated in year one.
