A long-term care facility in Middlesex is seeking a dedicated and flexible  Physical Therapist (PT ) to join our team on a per diem basis, with the opportunity to make a meaningful impact on residents’ mobility, independence, and quality of life. 

Currently hiring for  Per diem. 

Responsibilities: 

• Evaluate residents and develop individualized treatment plans  

• Provide therapy to improve mobility, strength, and functional abilities  

• Collaborate with interdisciplinary team members to support optimal outcomes  

• Document patient progress in accordance with facility standards  

• Educate residents and families on treatment plans and exercises  

• Ensure compliance with safety and regulatory requirements

Qualifications : 

• Active Physical Therapist license in the State of New Jersey  

• Prior long-term care experience required  

• Strong clinical, communication, and documentation skills  

• Ability to work independently and as part of a team  

• Flexible availability, including weekdays and weekends
