You will be paid for each completed patient visit (with documentation). WORK INDEPENDENTLY - 1-to-1 IN-HOME PATIENT CARE Are you a dedicated individual with a heart for compassion and kindness? Responsibilities Perform home visits for patients in a home care setting Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our patient's plan of care Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition Qualifications Current license as an Occupational Therapist in Connecticut Valid driver’s license Reliable transportation and willing to travel within a designated service area Strong communication and interpersonal skills Home health experience including Medicare helpful but not required WHO WE ARE At CareCo Medical Home Health, we've provided quality in-home support services since 2004.
