Community Health Advocate: Connect to Care & Resources

MetroHealth is seeking a Community Health Worker to support patients of the assigned clinic and surrounding community by promoting health, reducing disparities, and improving access to care. You will screen for social determinants of health and connect at-risk patients with eligible services.
