- Extensive experience with surgical patient population and telemetry. - Prior VAD experience and certification. - Credentials: License verification required through Nursys. - Checklist/Testing required for onboarding: Ascension Pharmacology Exam, Ascension Medical Surgical Exam, Ascension Core Competencies, Ascension EKG Exam, and Ascension Skills Checklist. - Personal health history requirements for onboarding: Ascension Color Vision, MMR, Fit Test, Hepatitis B, Health Statement, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, and Tdap. - Professional requirements for onboarding: Ascension Onboarding Requirements Education Attestation, Ascension Nursing Contract Worker Onboarding, Ascension Badge Photo, Driver's License/State ID, Ascension Profile, Ascension 2 Professional References, Ascension Criminal Background Check Results, Ascension EPLS, Ascension Education Verification, Ascension Confirmation of Assignment, Ascension Employment Verification. - Optional personal health and professional documents: Influenza Vaccine, Positive PPD-Negative Chest X-Ray and Annual TB Questionnaire, Hiring Manager Evaluation, Ascension Name Change Document.
