In-Home Health NP/PA Clinician (Per Diem)

Signify Health, part of CVS Health, seeks Per Diem Clinicians (NPs or PAs) to perform in-home health evaluations for members in their own residences. You’ll meet for about 45 minutes, review medications and history, perform a physical, and coordinate with health plans. Travel locally within licensed states as needed.
