In this role, you will help provide high‑quality, patient‑centered care for residents and staff in the Tele Med Surg specialty. Palos Hospital is not considering external ADN candidates for these RN openings. Internal ADN candidates are encouraged to apply. Job Overview The registered professional nurse assumes accountability and responsibility for the individualized, age‑specific care of a patient from admission to discharge. The nurse provides direct care, assists with other patient care as needed, collects and analyzes assessment data to identify nursing diagnoses, collaborates with the patient, family and healthcare team, develops a plan of care, provides evidence‑based education, and evaluates the quality and effectiveness of nursing practice. Responsibilities Assess and record patient status and identify care needs upon admission and at points of care. Organize assessment health data to ensure it is accurate, complete and accessible. Identify problems that require immediate attention. Validate diagnoses with patient, family and team members. Document diagnoses and anticipated outcomes and plan of care. Initiate problem lists at admission and update them as needed, incorporating age‑specific care principles. Confirm that the individualized plan of care has been discussed and agreed upon by the patient or family. Begin discharge planning at admission and consult with patient, family and team regarding the discharge plan. Identify teaching needs on admission and during the hospital stay. Arrange conferences with patient or family and other team members when indicated. Prioritize and organize evidence‑based nursing interventions aligned with standards of care. Provide individualized patient and family education based on evidence‑based practice. Document all assessments, interventions and patient responses. Continuously review the patient’s health data and update the plan of care. Ensure revisions to the plan are systematic, ongoing and driven by realistic, measurable goals. Document progress toward identified outcomes. Identify unmet outcomes and direct continuity of care. Reassess nursing actions for effectiveness in achieving outcomes. Communicate with patient, family and team regarding care and nursing role. Collaborate on the patient’s progress and plan of care. Present relevant patient information at multidisciplinary meetings for peer feedback. Make appropriate referrals for continuity of care. Required Qualifications Bachelor of Science in Nursing (BSN) Current State of Illinois Registered Professional Nurse license American Heart Association Basic Life Support (BLS) certification Strong interpersonal, collaboration and analytical skills Associate Degree in Nursing (ADN) acceptable with commitment to obtain a BSN within three years of employment. Additional requirements for specific areas: Emergency Room: ACLS, PALS/ENPC & TNCC within one year Pediatric Emergency Room: Pediatric Advanced Life Support (PALS) & TNCC within one year Non‑Invasive Cardiology: ACLS Preferred certifications: Trauma Nurse Specialist (TNS), Emergency Communications, Certified Pediatric Nurse, Certified Emergency Nurse Preferred Qualifications Experience in a specific clinical area.
