Position Summary Under the general supervision of the Director of Pharmacy Services, the Clinical Pharmacist will assume responsibility and accountability for the management of rational antibiotic therapy used within the Hospital, or management of the critical care or high‑risk patients as defined by area of responsibility. The pharmacist will provide collaboration and consultation with other health professionals on the therapeutic use of antibiotics or other medications for patients under the scope of responsibility, and will also provide education and training for health professionals involved in this clinical forum. Essential Functions and Responsibilities Understands and adheres to the Hospital Performance Standards, Policies and Behaviors. Improves the monitoring and management of the clinical pharmacy antibiotic service or critical care/high‑risk groups to include interventions that improve quality and cost‑effectiveness of patient care. Promotes the clinical pharmacy development of pharmacy staff by providing clinical leadership and education as appropriate. Works collegially with others in all activities to achieve organizational objectives, cooperating in interpersonal and team relationships to establish common ground, build shared goals, and encourage new ideas. Generates creative approaches and new ideas to improve performance, service, and quality, including identifying options and involving appropriate people in decision‑making. Takes personal responsibility for all work activities and actions, following through on commitments, and acknowledging mistakes or failures without blaming others. Demonstrates understanding and sensitivity toward others by treating people with courtesy and respect, establishing positive relationships and a pleasant manner. Performs all other related duties as assigned. Education and Experience Requirements Graduation from an accredited School of Pharmacy with a PharmD degree. Current registration as a pharmacist in New York State. Role Injection The Clinical Pharmacist will join four existing clinical pharmacists. Essential Criteria Possession of a PharmD degree. Active New York State pharmacist registration. Completion of PGY1 residency with board certification; PGY2 residency preferred. Clinical Experience Experience in rounding with CCU/ICU patients. Counseling for CHF/COPD patients. Investigation and reporting of medication errors. Responsibilities Monitoring and follow‑up with Sentri7. Handoff and follow‑ups from morning shift clinical pharmacists. Providing staff education and support. Core Competencies Ability to interpret, understand and communicate effectively in written and verbal English. Ability to act in a professional capacity and serve as a role model. Retains composure under stress. Physical/Mental Demands and Work Environment Must be physically able to ambulate or be stationary for extended periods of time, perform difficult manipulative arm‑hand steadiness and hand‑eye coordination skills. Must be physically able to meet all regulatory requirements. Ability to focus and concentrate on fine detail with interruption and attend to tasks for more than 60 minutes at a time. Individual must be able to understand theories behind several related concepts and remember multiple tasks given during the course of a day. Must be able to detect, interpret and judge objects both closely and at a distance. May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital's SDS (Safety Data Sheet) database and may be accessed through the hospital's Intranet site (Employee Tools / SDS Access). A copy of the SDS database can also be found at the hospital switchboard, saved on a disc.
