Field Services SpecialistThe Field Services Program is responsible for linking people with HIV (PWH) to medical care, primarily those who are out of care. The Field Services Specialist (FSS) is a community outreach position within the Philadelphia Department of Public Health (PDPH), Division of HIV Health (DHH). This position reports to the Field Services Program Coordinator. The core functions of the FSS will be done through meeting clients in their homes and in the field, as well as accompanying patients to medical visits and community organizations. Much time is spent talking with the clients, building trust, determining what is preventing them from accessing medical care, and coordinating services. The FSS will also be responsible for providing outreach services to people who are newly diagnosed with HIV and individuals who may be impacted by community outbreaks of HIV. The Field Services Specialist will be required to work closely with partners in the HIV community, including medical providers, community-based organizations, and various divisions within the local and state health departments. The FSS must have a valid driver's license, a good driving record, and willingness to transport patients, coworkers, and other pertinent personnel. Hired employees will not be required to have a car. Occasional evening work is required.Responsibilities:Communicate with management team, other health department staff, and public health partners in a timely and professional mannerConsult with physicians, nurses, and other health professionals to ensure patients connect to medical careDemonstrate the ability to work independently while out in the fieldEffectively locate people living with HIV who are newly diagnosed and/or out of medical care through home and field visits, phone calls, social media platforms, and public databasesNavigate the city of Philadelphia geographical area effectivelyParticipate in coordinated response activities during public health emergencies, such as cluster and/or outbreak responseParticipate in training and career development seminars including ARTAS training, HIV Counseling and Testing, confidentiality and security, and professional ethics trainingPerform other related duties required by the Department of Public Health and/or Division of HIV HealthPrioritize tasks for multiple concurrent projects, track the progress and status of tasks in detail, and provide status updates on short noticeProvide appropriate substance abuse and mental health assessmentProvide supportive services referrals to address patient/client barriers and needsWork with PWH to support positive coping strategies, identify barriers to accessing medical care, and navigate community resourcesSkills:Willingness to conduct core outreach activities through home visits, doctor's visits, and other field visitsDesire to work with people with HIV (PWH) and people who inject drugs (PWID), communities of color, and LGBTQ peopleAbility to work effectively and respectfully with people of all gender identities and sexual orientations, applying a practice of cultural humility and ongoing self-reflectionAbility to perform all specified duties and tasksAbility to solve complex problems quickly and effectivelyAbility to work collaboratively with a teamBasic computer skills with knowledge of Microsoft Word and Microsoft ExcelCompetent in written and oral communicationCourteous customer service is vital to the positionDemonstrate effective organization and time management skillsDemonstrate respect for the confidentiality and privacy of health dataFlexibility, professionalism, and the ability to remain calm in a fast-paced environment are essential attributesInteract sensitively, effectively, and professionally with people from diverse cultural, socioeconomic, and educational backgroundsBuild positive working relationships with team members and support a healthy office cultureKnowledge of HIVWillingness and flexibility in accepting directionQualifications:High school diploma or equivalent requiredMinimum of two years conducting outreach and/or community engagement activities with communities of color and/or persons who inject drugs and/or LGBTQ individualsAt least one year working in an HIV, health services, social services, public health, or community-based organizationValid driver's license requiredSalary: Starting salary of $51,000 per yearPHMC is an Equal Opportunity and E-Verify Employer.
