You will review records, verify insurance, schedule appointments with specialists, communicate referral details to patients, and document all actions in the EHR. You will follow up on referral outcomes, resolve issues that arise, and maintain strong relationships with community providers to support smooth coordination across the care continuum. Who YOU ARE Minimum 1 year of experience in the Healthcare field. Familiar with Electronic Health Records Bilingual English/Spanish is a plus Able to travel between Dallas clinic locations & stay Up‑to‑date on required immunizations and testing. #J-18808-Ljbffr
