In this role, you will play a vital part in coordinating comprehensive patient care plans, balancing direct patient interaction with administrative responsibilities. Frequent travel to patient homes and effective use of both electronic and paper-based documentation systems are key aspects of this position. Responsibilities

• Develop and implement individualized care plans to achieve optimal health outcomes
• Provide health education and counseling to patients and families to promote wellness
• Collaborate closely with interdisciplinary teams to coordinate patient services
• Maintain clear and effective communication with patients, families, and healthcare providers
• Manage time efficiently to balance direct patient care with administrative duties
• Monitor patient progress and adjust care plans as necessary
• Accurately document patient information using electronic health records and paper systems
• Support team coordination within a small group of healthcare professionals

 

Required Qualifications

• Minimum 2 years of experience in healthcare coordination
• Healthcare Administration, or a related field

 

Preferred Qualifications

• Experience in developing comprehensive care plans
• Strong skills in patient education and interdisciplinary collaboration
• Excellent communication and organizational abilities
• Proven teamwork and time management skills
